Adolescent Assessments

Sliding fee scales are available for those who qualify, based on income and family size. No one is denied services due to inability to pay. Ability to pay is based on sliding fee scale for those who qualify.

ADOLESCENT WALK-IN CLINIC  FOR COUNSELING SERVICES

Evaluations are available for adolescent under the age of 18 on a first come/first served basis on
Monday at 12:00 P.M or 1:30 P.M
Tuesday at 12:00 P.M or 1:30 P.M and
Thursday at 8:00 A.M or 9:30 A.M

If the first session is full you should receive a spot in the second session. Plan for 2- 2 ½ hours. Patients under the age of 18 will be scheduled. Please call. Assessments will occur face to face so masks are required.

A friend or family member should accompany you. Please allow AT LEAST 2 1/2 hours for this process. Some types of evaluations require payment before you can be seen by a counselor. Please call ahead for that information.

ADOLESCENT WALK-IN CLINIC MEDICATION/PRESCRIBING SERVICES

Evaluations are available for adults over the age of 18 on a first come/first served basis on
Tuesday and Wednesday at 8:00 A.M and 9:00 A.M.
(Cutoff times are 8:30 A.M. and 9:30 A.M.)
Please bring health insurance or Title XIX card and proof of income (paystub or tax return), medications list.

PAYMENT IS EXPECTED AT THE TIME OF SERVICE
