Please note! Assessments are for therapy services. If you are seeking medication management, refer to the medication management page.

Interpretive Services Available Upon Request Or Identified Need

At the time of initial evaluation, the cost of our services will be explained to you.  Some patients may be eligible for a sliding fee schedule.  It is our expectation that assigned fees are paid at the time of services.  We also will consider payment plans and bill you accordingly.  Patients have the right to services regardless of the ability to pay.

WALK-IN CLINIC assessments for adults

Available in the Mason City Office only.

Evaluations are available on a first come/first served basis on the following days/times:

Monday: 12:30 P.M. or 2:00 P.M.
Tuesday: 8:30 A.M., 10:00 A.M, 12:30 P.M., or 2:00 P.M.
Thursday: 8:30 A.M. or 10:00 A.M.

Please bring health insurance or Title XIX card, proof of income (paystub or tax return), and list of current medications.  A friend or family member may accompany you.  Please allow at least 2 hours for this process.

 

outreach office assessments for adults

These assessments are scheduled by appointment in the following offices: Charles City Office, Forest City Office, Hampton Office, and Algona Office.

Please bring health insurance or Title XIX card, proof of income (paystub or tax return), and list of current medications. A friend or family member may accompany you. Please allow at least 11/2 hours for this process.

 

children and adolescent assessments

These assessments are scheduled by appointment in all offices, including the Mason City office.

Please bring health insurance or Title XIX card, proof of parent/guardian income (paystub or tax return), and list of current medications. A parent or guardian should accompany the child or adolescent to the assessment.  Please allow at least 2 hours for this process.
