For any former employees that were enrolled in any of the group health plans offered by Prairie Ridge during calendar year 2024, form 1095-C (Employer-Provided Health Insurance Offer and Coverage) is available upon request. Your request should be submitted to Mike Huisman, CFO, via email (mhuisman@prairieridge.net), USPS mail at the mailing address for Prairie Ridge, or by calling (641) 243-7216. Your form will be delivered to you within 30 days of the date your request is received. Please indicate in your request how you would like your form 1095-C delivered to you.
