Admission Criteria:

Eligible patients need a diagnosis of schizophrenia, schizoaffective disorder, bipolar disorder, or severe MDD. They can have co-occuring substance use disorders but this cannot be their primary reason for recurrent hospitalization. They also need to be Medicaid eligible, live within 25 miles of Mason City, age >18, and one of the following:

-Two or more inpatient admissions in past 24 months
-Greater than 3 weeks of hospitalization in the past 12 months
-Greater than 3 months of residential care in the past 12 months
-Decompensation or high risk of decompensation with traditional treatment due to treatment noncompliance or severe life stress

What is ACT?
Assertive Community Treatment (ACT) is an evidence-based model of treatment for individuals with severe and persistent mental illness for whom traditional ap

proaches have been ineffective.

ACT team is staffed by physicians, nurses, social workers, substance use counselors, mental health counselors, vocational specialist and peer support specialists

-24 hour/7 day a week access to staff
-Person centered treatment planning
-Medication management and psychiatric services
-Symptom management and skills teaching
-Family support and education
-Substance use reduction services
-Physical health screening and care coordination
-Employment and housing assistance
-Assistance maintaining activities of daily living
